Online Math Assessment 
Terms and Conditions Agreement

By scheduling this free assessment, I acknowledge and agree to the following:

• Intention to Enroll: This free assessment should be scheduled only for students whose families intend to enroll in Ellipsis Math programs. This ensures we can dedicate the proper time and attention to each student.
  
• Single Assessment per Student: Each student is allowed to take only one assessment. Duplicate registrations will be automatically canceled, and only the latest registration will be kept.
  
• Assessment Format: The assessment is online and lasts approximately 60 minutes.
  
• Purpose of the Assessment: The goal is to evaluate students’ current math skills and determine their level within Ellipsis Academy math programs.
  
• Test Privacy and Scoring: We do not release the test questions, answers, scores, or other test analytics. Students’ math skills are assessed through analysis of various areas appropriate to their level, and the numeric score is not relevant.
  
• Confirmation of Understanding: By submitting this form, I confirm that I have read and understand the above terms and that I am scheduling this assessment with the intention to enroll my student in Ellipsis Math programs.
